Turbotax
Although Intuit is aware of a glitch in their Turbotax software, that allows the IRS to withdraw funds from accounts, they have failed to be proactive in warning people about this problem.
Allegedly, if you hit yes to auto withdrawal, and then change you mind and click back and click no, the new choice doesn't register. People are spending hundreds of hours trying to recover their money, and trying to find out the source of the problem.
